Study: CT-Based Computer Assistance Can Improve Pedicle Screw Accuracy

CT-based computer-assisted surgical navigation systems can help surgeons more accurately place pedicle screws during spine surgery, according to a study published in Spine. Researchers examined 100 patients with lumbar degenerative spondylolisthesis who underwent surgery using a CT-based computer guidance system. The patients either underwent a preoperative CT scan or an intraoperative CT scan for navigation.

Of the 504 screws placed, 471 were correctly inserted into the pedicles. The preoperative CT scan group had 91.8 percent accuracy, and the intraoperative CT scan group had 95.2 percent accuracy. There was an overall perforation rate of 6.5 percent.

There were 33 screws misplaced, but only one was replaced. Surgical time was shorter for the intraoperative CT scan group due to the automatic recognition and merging of the images with the patient's anatomy, avoiding registration with a paired-point technique.
